CIS Computing Facilities Student Account Application

Please read the following information carefully before completing this form. Doing so may save you from unnecessary delays.

DO NOT COMPLETE THIS FORM IF:

You are not taking classes in the CIS department. Each application is manually verified, and you will not receive an account if you are not eligible.
You have not yet registered for your courses. You must register first, and then wait 72 hours, to ensure that your name appears on the registration report that is used to verify your application.
You don't have a BlazerID yet. All students must first get a BlazerID before applying for a CIS account. Please visit http://www.uab.edu/blazerid to do so. If you feel this is not possible, please contact the CIS Helpdesk at helpdesk@cis.uab.edu.
You are not registered for CS105 or higher. This form is for students who are enrolled in CS105 or higher. CS101 students will create their accounts on the first day of class. Students that are not currently enrolled in any CIS courses will not receive an account.
You already have a CIS account and have forgotten your password. Instead, use the CISV tool to reset your password, at https://www.cis.uab.edu/cisv/
You previously took CS101 and now you are now registered for CS105 or higher. Instead, email helpdesk@cis.uab.edu and ask for an account conversion from CS101 to undergrad. Be sure to include your BlazerID.
You are not sure whether you have a CIS account already.. Instead, please use the CISV tool at https://www.cis.uab.edu/cisv/ to determine whether you have a CIS account.
