CIS Computing Facilities Student Account Application

These accounts are for students taking CS105 or above. All applications are verified by CIS staff. Do not apply if you do not qualify - you will not be issued an account.

If you previously took CS101 and are now taking CS105 or above, do not complete this form. Instead, send a note to helpdesk (at) cis (dot) uab (dot) edu to request that your CS101 account be converted to a standard CIS account.

If you have forgotten your password, do not complete this form again. Please contact the CIS Helpdesk for a password reset.

A BlazerID is required before you may apply for a CIS account. To obtain a BlazerID from UAB, visit http://www.uab.edu/blazerid and complete the online process.

Note about CIS email: You will get a CIS email address of the form BlazerID@cis.uab.edu. However, this email will be set up to automatically forward to your Blazer email BlazerID@uab.edu. If you wish to have the email stay on CIS systems instead, you may edit your UNIX forward file. If you do this, you will be responsible for checking BOTH email accounts for CIS-related email, since many of UAB's administrative systems will email directly to your Blazer email, not to CIS.

CIS accounts do -not- use your BlazerID password. This is a common cause of confusion. Initially they use the password you provide in the fields below. You may change this in the future.
